What is a fire extinguisher inspection tag and record?

A fire extinguisher inspection tag and record is the document that proves each portable fire extinguisher on site has been checked, is charged, and is ready to use. It captures the extinguisher location and serial number, the type and rating, the inspection date, the result of each routine check, the name of the person who did it, and the date the next inspection is due. Used as a multi-unit record it lets one competent person walk a site, tick each extinguisher off in turn, and leave behind a dated log covering a full twelve months of checks and service.

Under AS 1851-2012 portable fire extinguishers get a routine service at least every six months by a competent person, with more thorough yearly checks and a five-yearly overhaul and pressure test. A quick in-house inspection (present, accessible, gauge in the green, pin and seal intact) can be done monthly and is distinct from that six-monthly service. A consistent tag and record keeps the history in one place so a fire warden, building owner or auditor can see at a glance that every unit is in date and nothing has been missed. It also supports the general duty under the model Work Health and Safety (WHS) Act and Regulations (enacted in all states and territories except Victoria, which has the Occupational Health and Safety Act 2004 (Vic) and OHS Regulations 2017 (Vic)) to provide and maintain safe plant and emergency equipment, and gives fire authorities clear evidence that the site's first-response gear is maintained rather than assumed.

Benefits of using this fire extinguisher inspection tag and record

  • Complete coverage: a single multi-unit record lets you tick off every extinguisher on site in one round rather than chasing loose tags.
  • AS 1851-2012 evidence: dated routine service entries show servicing happened at the required intervals for auditors and insurers.
  • Nothing expires unnoticed: a next-due date against each unit means a flat or out-of-date extinguisher is caught before it is needed.
  • Clear accountability: the inspector's name against every check means each entry has a person standing behind it.
  • Faster warden rounds: a fixed row-per-unit layout turns the monthly walk into a quick, repeatable task instead of guesswork.
  • Incident readiness: a maintained record is strong proof the first-response equipment was serviceable if a fire ever breaks out.

What to include in a fire extinguisher inspection tag and record

This fire extinguisher inspection tag and record covers 10 key areas:

  • Extinguisher location or point number, so each unit maps to a fixed spot on site
  • Serial number and manufacture or last-test date for the individual cylinder
  • Extinguisher type and fire rating (water, foam, dry powder, CO2, wet chemical)
  • Inspection date and the interval being recorded (routine six-monthly or monthly visual)
  • Pressure gauge reading in the operable green zone, or a pass/fail against it
  • Pin, tamper seal and safety tag present and intact
  • Hose, nozzle and body free of damage, corrosion, dents or blockage
  • Signage and access clear, unit correctly mounted and visible
  • Weight check against charged weight where applicable
  • Result, inspector name or number, and the next inspection due date

How to use this fire extinguisher inspection tag and record

  1. List every extinguisher on site: Before the first round, walk the building and record every portable extinguisher with a location and serial number so each has its own row. A complete list is what turns a stack of individual tags into an auditable record you can check for gaps.
  2. Check pressure, pin and seal: At each unit confirm the pressure gauge sits in the operable green zone, the pull pin is in place, and the tamper seal is unbroken. A dropped gauge needle or a broken seal means the unit is taken out of service and flagged for a full service straight away.
  3. Inspect the body, hose and access: Look over the cylinder, hose and nozzle for dents, corrosion, cracks or blockages, and confirm the extinguisher is mounted, signed and clear of obstruction. Any physical damage or blocked access is recorded as a fail so it is fixed rather than left for the next round.
  4. Record the result and inspector: Mark each unit pass or fail, note anything that needs attention, and sign or number the entry against the inspection date. A named result on every row is what makes the record objective evidence rather than an unverifiable tick.
  5. Set the next-due date and act on fails: Enter the next inspection date for each unit so the routine interval is visible, and raise any failed extinguisher for repair, recharge or replacement. Do not close the round until every fail has a follow-up action logged against it.

In MapTrack, you can manage fire safety inspections and equipment checks digitally. Each submission is stored as a timestamped PDF against the asset record.

How often should you complete this form?

Have a routine service done on every portable extinguisher at least every six months as set out in AS 1851-2012, with a quick monthly in-house inspection in higher-risk areas to confirm each unit is present, charged and unobstructed. More thorough yearly checks and a five-yearly overhaul and pressure test fall due at longer intervals depending on the extinguisher type.

Use one record sheet per site and add a dated entry each time a round is done, so a single page builds a rolling twelve-month history. Book the six-monthly service and the longer-interval overhauls and pressure tests with a competent serviceperson and note the date they were completed against each unit.

Frequently asked questions

AS 1851-2012 sets a routine service at least every six months for portable fire extinguishers, covering the pressure, pin, seal, hose and physical condition checks on this record. Longer-interval activities such as pressure testing and overhaul fall due at set periods depending on the extinguisher type, typically five yearly for a pressure test. Many sites also add a monthly in-house inspection in higher-risk areas, which is distinct from the six-monthly service. The model WHS duty to maintain safe plant and emergency equipment sits behind all of this, and a dated record is how you show the intervals were met.

A routine six-monthly service under AS 1851-2012 should be carried out by a competent person who has been trained in the checks and knows how to identify a defect and take a unit out of service. Simple monthly in-house inspections that a unit is present, charged and unobstructed can be done by a trained staff member or fire warden. Pressure testing, overhaul and refilling are specialist tasks for a qualified serviceperson with the right equipment. Record whoever did the work by name or number against each entry.

A monthly in-house inspection is a quick confirmation that the extinguisher is in its place, the gauge is in the green, the pin and seal are intact, and access is clear. It does not replace the six-monthly service. A six-monthly routine service under AS 1851-2012 is more thorough: it includes the visual checks plus a weight check where applicable, a closer inspection of the hose, nozzle and body, and formal recording by a competent person. Use this record for the six-monthly service and keep the monthly inspections as a lighter round on the same sheet if it suits your site.

Take the unit out of service immediately, tag it clearly as not to be used, and record the fault on the sheet. A flat gauge, broken seal, corrosion, damage or a missing pin all mean the extinguisher cannot be relied on. Arrange a replacement or a spare for that location so the area is still covered, and book the failed unit for recharge, repair or disposal with a servicing technician. Do not close out the inspection round until every failed unit has a follow-up action logged against it.

Yes, it is completely free. Open it in your browser, then use Print and choose Save as PDF. You do not need a MapTrack account. If you want to move beyond paper, MapTrack keeps a digital asset history for every extinguisher, schedules the next inspection automatically, and lets your warden run mobile inspections from a phone with photos attached.
