Mining templates

Free PDF-ready safety checklists and forms for surface and underground mining: mining and general JSAs, take 5 risk assessments, permits to work and confined space entry permits, pre-start checklists for drill rigs, loaders and excavators, haul truck inspections and gas detector calibration logs. It also has incident, near miss, hazard observation and spill reports, corrective action forms and toolbox talk records. Most templates cite Australian WHS laws, some also cite state mining safety laws, and US MSHA checklists for mobile equipment and underground vehicles are included too. Mining rules differ by country and state, so check each form against the law where you operate and your mine's safety management system. Download and use straight away, or digitise in MapTrack.

Frequently asked questions

What safety templates do mining operations need?
It depends on the mine, the jurisdiction and your safety management system, so no single list fits every site. This collection covers everyday safety paperwork: JSAs and take 5s before a task, pre-start checklists for mobile plant, permits to work and confined space entry permits, gas detector bump test and calibration logs, toolbox talk records, and incident, near miss and corrective action forms. Check each form against your mine's rules and the law where you operate before you rely on it.
Are these templates suitable for underground mining?
Some are written for it. The mining JSA form prompts for ground support, falls of ground, dust and ventilation, and lists a self-rescuer as PPE for underground work. The confined space entry permit covers ventilation, atmospheric testing and rescue, and the underground vehicle inspection checklist cites MSHA's rules for underground coal mines (30 CFR Part 75). General forms such as the take 5, toolbox talk record and incident reports work above or below ground.
Can I digitise these mining templates in MapTrack?
Yes. Build each form in MapTrack's form builder and link it to the relevant assets. Your crew scans the asset's QR code, completes the form on their phone and adds photos, and each completed form is stored against the asset with a record of where the check was done.
Are the templates free to download?
Yes. All mining templates are free to download and use. Enter your email address to access the PDF-ready template. No MapTrack account required. If you later want to digitise your mining forms, we offer a free 30-day trial.
What does MSHA require for mobile equipment inspections?
At US surface and underground metal and nonmetal mines, 30 CFR 56.14100 and 57.14100 require self-propelled mobile equipment to be inspected by the equipment operator before it is placed in operation on a shift. Safety defects must be corrected in a timely manner, equipment that is hazardous to run must be taken out of service or tagged until it is fixed, and safety defects not corrected immediately must be reported to and recorded by the mine operator. The MSHA equipment inspection checklist here cites these sections.
