What is a vehicle annual service record?
A vehicle annual service record is a structured form completed by a mechanic or service provider each time a vehicle undergoes scheduled maintenance. It documents exactly what was inspected, checked, replaced or adjusted during the service, including engine oil and filter, air filter, fuel filter, cabin filter, coolant, transmission fluid, differential oil, power steering fluid, brake fluid, brake pads/shoes, brake discs/rotors, tyres, battery, belts, spark plugs (if petrol), wiper blades, lights, suspension, steering, exhaust and A/C system. It also records parts used, fluids used, the next service due date and odometer, any recommendations or additional work required, and mechanic and fleet manager sign-off. The completed record provides a full maintenance history for the vehicle.
For fleet managers, the annual service record is the most important document in a vehicle's life. It provides the definitive evidence that scheduled maintenance was performed, what was found, and what was done about it. A complete set of annual service records protects warranties, supports insurance claims, increases resale value and demonstrates duty of care under WHS and Chain of Responsibility obligations. Under the Australian Road Rules and state roadworthiness legislation, vehicles must be maintained in a condition that does not endanger other road users, and a documented service history is the primary evidence of compliance. For heavy vehicles, the NHVR Maintenance Management System further requires that maintenance records are retained and producible on request. When paired with a digital platform like MapTrack, these records become instantly searchable and auditable across the entire fleet.

Benefits of using this vehicle annual service record
- Maintenance history: a complete record of every service event, what was done, and what parts and fluids were used.
- Resale value: vehicles with a full service history typically command a higher resale price and are easier to sell.
- Warranty compliance: demonstrate that scheduled services were completed on time to protect manufacturer warranties.
- Fleet planning: track when each vehicle is next due for service, plan budgets for parts and labour, and identify vehicles with recurring issues.
- Compliance and audits: provide documented evidence that fleet vehicles are maintained to a safe standard, supporting WHS and insurance obligations.
- Cost tracking: record parts, fluids and labour for each service to understand true cost of ownership per vehicle.

What to include in a vehicle annual service record
This vehicle annual service record covers 9 key areas:
- Vehicle details: registration, make/model, year, VIN/chassis, odometer at service, fleet number.
- Service details: service date, service type (minor/major/scheduled), service provider, mechanic name.
- Service items table: with columns for Checked, Replaced, Adjusted, N/A and Notes: engine oil and filter, air filter, fuel filter, cabin/pollen filter, coolant (check/flush), transmission fluid, differential oil, power steering fluid, brake fluid, brake pads/shoes (thickness), brake discs/rotors, tyres (tread depth, rotation, alignment), battery (test/replace), belts (drive, timing, A/C), spark plugs (if petrol), wiper blades, lights, suspension, steering, exhaust system, A/C system (gas check/recharge).
- Parts used table: part description, part number, quantity.
- Fluids used table: fluid type, brand, volume.
- Next service due: date, odometer, service type.
- Recommendations / additional work required: .
- Mechanic sign-off: signature, name, date.
- Service manager / fleet manager approval: - signature, name, date.
How to use this vehicle annual service record
- Fill in the vehicle details (registration, make/model, year, VIN/chassis, odometer, fleet number) and service details (date, type, provider, mechanic) at the top of the form.: Verify the odometer reading is accurate by cross-referencing with GPS or fleet management data. Confirm the service type (minor, major or scheduled) matches the interval for this vehicle at this mileage.
- Work through each service item. For each item, tick the applicable column(s). Checked, Replaced, Adjusted or N/A. Use the Notes column for measurements (e.g. brake pad thickness, tyre tread depth) or observations.: Record objective measurements wherever possible. For example, note brake pad thickness in millimetres and tyre tread depth rather than subjective assessments like good or fair. This creates quantifiable data for trend analysis.
- Record any parts used in the parts table - description, part number and quantity.: Include the manufacturer part number for each component used. This supports warranty claims if a part fails prematurely and enables accurate reordering for future services.
- Record any fluids used in the fluids table. Type, brand and volume.: Note the oil grade (for example 5W-30 or 15W-40), brand, and exact volume in litres. This information is critical for warranty compliance and cost tracking per vehicle.
- Set the next service due - date and odometer reading - based on the manufacturer's service schedule or fleet policy.: Calculate the next due odometer by adding the interval distance to the current reading. Set the calendar date based on expected usage rate. Enter both values so the fleet system can trigger a reminder on whichever threshold is reached first.
- Note any recommendations or additional work required that was identified during the service but not completed (e.g. timing belt replacement due at next service).: List each deferred item with a clear reason and recommended completion date. This creates a formal follow-up register that prevents identified issues from being forgotten between service intervals.
- The mechanic signs and dates the form to confirm the work was completed.: The mechanic signature confirms that all items marked as checked, replaced or adjusted were completed to an acceptable professional standard and that no unresolved safety concerns remain.
- The service manager or fleet manager reviews and signs to acknowledge the service and any recommendations.: The reviewer verifies that all required service items were addressed, reviews deferred work notes and recommendations, confirms the next service due date and odometer are recorded, and approves the record for filing.

Back to download formHow often should you complete this service procedure?
Most vehicles should follow the manufacturer's scheduled service intervals, typically every 10,000 to 15,000 km or 6 to 12 months, whichever comes first. A minor service (oil and filter change, fluid checks, visual inspection) is usually performed at shorter intervals, while a major service (comprehensive inspection of brakes, suspension, steering, drivetrain, belts and filters) is performed at longer intervals, often every 40,000 to 60,000 km or every 2 to 3 years. Fleet vehicles in demanding conditions (dust, heavy loads, frequent stop-start driving, towing) may require more frequent servicing. Always check the manufacturer's service schedule and your fleet policy. In MapTrack, you can set service schedules based on date or odometer and receive automatic reminders.
