What is a vehicle annual service record?

A vehicle annual service record is a structured form completed by a mechanic or service provider each time a vehicle undergoes scheduled maintenance. It documents exactly what was inspected, checked, replaced or adjusted during the service, including engine oil and filter, air filter, fuel filter, cabin filter, coolant, transmission fluid, differential oil, power steering fluid, brake fluid, brake pads/shoes, brake discs/rotors, tyres, battery, belts, spark plugs (if petrol), wiper blades, lights, suspension, steering, exhaust and A/C system. It also records parts used, fluids used, the next service due date and odometer, any recommendations or additional work required, and mechanic and fleet manager sign-off. The completed record provides a full maintenance history for the vehicle.

How to complete a vehicle service record

  1. Fill in the vehicle details (registration, make/model, year, VIN/chassis, odometer, fleet number) and service details (date, type, provider, mechanic) at the top of the form.
  2. Work through each service item. For each item, tick the applicable column(s). Checked, Replaced, Adjusted or N/A. Use the Notes column for measurements (e.g. brake pad thickness, tyre tread depth) or observations.
  3. Record any parts used in the parts table - description, part number and quantity.
  4. Record any fluids used in the fluids table. Type, brand and volume.
  5. Set the next service due - date and odometer reading - based on the manufacturer's service schedule or fleet policy.
  6. Note any recommendations or additional work required that was identified during the service but not completed (e.g. timing belt replacement due at next service).
  7. The mechanic signs and dates the form to confirm the work was completed.
  8. The service manager or fleet manager reviews and signs to acknowledge the service and any recommendations.

Most vehicles should follow the manufacturer's scheduled service intervals - typically every 10,000–15,000 km or 6–12 months, whichever comes first. A minor service (oil and filter change, fluid checks, visual inspection) is usually performed at shorter intervals, while a major service (comprehensive inspection of brakes, suspension, steering, drivetrain, belts and filters) is performed at longer intervals, often every 40,000–60,000 km or every 2–3 years. Fleet vehicles in demanding conditions (dust, heavy loads, frequent stop-start driving, towing) may require more frequent servicing.
