- Fill in the vehicle details (registration, make/model, year, VIN/chassis, odometer, fleet number) and service details (date, interval, provider, mechanic) at the top of the form.: Verify the odometer reading matches the expected interval. Cross-reference with the fleet schedule to confirm the correct service type is being performed for this vehicle at this mileage.
- Select the service interval being performed - 10,000 km (minor), 20,000 km (intermediate) or 40,000 km (major). Some items only apply at certain intervals (e.g. spark plugs at 40k).: Highlight or circle the interval being completed so the mechanic and reviewer can clearly see which scope of work applies. Items marked as 40k-only can be left as N/A during a 10k or 20k service.
- Work through each section. For every item, tick the applicable column. Checked, Replaced, Adjusted or N/A. Use the Notes column for measurements (e.g. brake pad thickness mm, tyre tread depth mm) or observations.: Record numerical measurements wherever possible rather than subjective assessments. For example, note brake pad thickness in millimetres and tyre tread depth at the shallowest point rather than writing good or OK.

A minor service (oil and filter change, fluid top-ups, visual checks) is usually performed at 10,000 km intervals. An intermediate service (filter replacements, brake inspection, tyre rotation) is performed at 20,000 km intervals. A major service (comprehensive inspection of brakes, suspension, steering, drivetrain, belts, spark plugs, coolant flush and transmission service) is performed at 40,000–60,000 km intervals. Fleet vehicles in demanding conditions - dust, heavy loads, towing, frequent stop-start driving - may require shorter intervals.
