An environmental spill report is a formal document used to record the details of a chemical, fuel or hazardous substance spill in the workplace or surrounding environment. The report captures the substance identity, estimated volume, spill location, containment and clean-up actions taken, affected areas (soil, water, drains), regulatory notifications made, waste disposal details and corrective actions to prevent recurrence.
Environmental spills can have serious consequences for worker health, the surrounding community and the natural environment. In Australia, the Environment Protection Act in each state and territory requires the person responsible for a spill to notify the relevant environmental authority (such as the EPA) if the spill causes or threatens material environmental harm. The WHS Act also applies because spills create immediate workplace hazards. - Contain the spill and protect workers from exposure.: Apply immediate containment using spill kits, bunding, absorbent booms or drain covers. Notification timeframes vary but are typically "as soon as practicable" after becoming aware of the spill. Record the notification date, time and reference number on the report.
- Complete the clean-up and document all waste disposal.: Clean up the spill using methods appropriate for the substance and the affected surface. Collect all contaminated materials, absorbents and PPE as waste. Classify the waste, arrange collection by a licensed waste transporter and document the receiving facility and disposal method.
- Complete the spill report, investigate root cause and implement corrective actions.: Fill in all sections of the report while details are fresh. Investigate the root cause of the spill and document corrective actions to prevent recurrence. Assign a responsible person and due date for each action. File the report and retain for at least seven years for regulatory compliance.

An environmental spill report must be completed for every spill or release of a hazardous substance, regardless of volume. There is no scheduled interval. The trigger is the occurrence of a spill. The report should be started immediately after the spill is contained and completed within 24 hours.
