What is a driver licence verification form?

A driver licence verification form is a structured document used to confirm that a person holding a vehicle key or operating a fleet vehicle possesses a valid, current driver licence appropriate for the class of vehicle they intend to drive. The form captures the driver name, licence number, issuing state or territory, licence class (e.g. C, LR, MR, HR, HC, MC), expiry date, any conditions or restrictions (such as corrective lenses, automatic only or medical conditions), and whether the licence was sighted and verified by an authorised representative.

In fleet operations, verifying driver licences is a fundamental duty of care under WHS legislation and a practical risk control under chain of responsibility obligations. Employers and PCBUs have an obligation to ensure that only appropriately licensed drivers operate their vehicles. This driver licence verification form covers 10 key areas:

  • Licence details: licence number, issuing state or territory (or country), licence class held, date of issue.
  • Expiry and status: licence expiry date, current status (current, expired, suspended, cancelled, disqualified).

How to use this driver licence verification form

  1. Ask the driver to present their current physical driver licence and record their full name, employee or contractor ID and department or project.: Confirm the identity of the person matches the photo on the licence. Record all details legibly on the verification form at the time of sighting, not from memory after the fact. Check for any visible signs of alteration or tampering.
  2. Record the licence number, issuing authority, licence class and date of issue from the face of the licence.: Check the licence class against the vehicle type the driver will operate. In Australia, a C class covers vehicles up to 4.5 t GVM, LR covers light rigid, MR covers medium rigid, HR covers heavy rigid, HC covers heavy combination and MC covers multi-combination. Record the exact class shown on the licence.
  3. Check the expiry date and confirm the licence is currently valid and not expired, suspended or cancelled.: If the licence expiry date has passed, the driver must not operate a fleet vehicle until they provide evidence of renewal. Where available, verify the licence status online through the relevant state or territory transport authority (e.g. Service NSW, TMR Queensland, VicRoads).
  4. Record any conditions or restrictions printed on the licence, including corrective lenses, automatic transmission only, medical conditions or interlock requirements.: Ensure that any conditions can be met in the vehicle the driver will operate. For example, if the licence is restricted to automatic transmission, the driver must not be assigned a manual vehicle. Record all conditions on the form and flag any that may require operational adjustments.
  5. Record the verification outcome (verified, not verified or conditional), sign the form and set the next review date.: The verifier must sign and date the form to confirm the licence was personally sighted and the outcome recorded accurately. Set the next review date based on your organisation policy, typically every 6 to 12 months. How often should you complete this form?

What happens if a driver licence expires and is not caught?
If a driver operates a fleet vehicle on an expired licence and is involved in an incident, the consequences can be severe. The driver may face fines and criminal charges for driving unlicensed. The employer may face prosecution under WHS and chain of responsibility legislation for failing to ensure the driver was appropriately licensed. Insurance claims may be denied or reduced if the insurer determines the driver was not legally authorised to operate the vehicle.
