How often should you complete this form?

Defect reports should be submitted immediately whenever a fault, deficiency or safety concern is identified during any activity including pre-start inspections, routine maintenance, periodic audits or normal operation. There is no fixed schedule for defect reporting because defects are event-driven not calendar-driven.

Under the Australian WHS Act a worker who becomes aware of a safety issue must report it to the PCBU as soon as practicable. Delaying a defect report can expose the business to regulatory action if the defect causes an incident. What is the difference between a defect report and a work order?
A defect report is the initial record of a fault or deficiency found during an inspection or operation. A work order is the instruction to repair or rectify that defect. Who is responsible for submitting defect reports under WHS law?
