What is a gemba walk checklist?
A gemba walk checklist is a structured prompt sheet that guides a leader through a go and see walk of the workplace, capturing what they observe about safety, quality, standard work, 5S and the flow of work. Gemba is a Japanese term meaning the real place, the shop floor, yard or job site where value is actually created. The checklist records where the walk took place, who was spoken to, the observations made against each theme, the questions asked of the people doing the work, and the follow-up actions agreed on the spot. It is a record of what leaders saw and heard, not a scorecard used to judge individuals.
A gemba walk is a lean management practice, not a legislated standard, and this template treats it as one. The point is to move decisions to where the work happens: leaders see the real conditions, hear the frustrations of operators and fitters first hand, and confirm whether the standard work people are meant to follow is actually workable. - Set the purpose and pick the area: Decide what this walk is about before you go, whether it is safety, quality, a specific process or a recent problem. Pick one area so the walk stays focused rather than trying to cover the whole site in a single pass.
- Go to where the work happens: Walk the actual floor, yard or job site rather than reviewing it from an office or a screen. Observe the process running normally and resist the urge to fix things on the spot until you have finished watching and understanding what is happening.
- Watch, then ask open questions: Observe the work against the checklist themes first, then ask the people doing it open questions about what makes the job hard, what slows them down and what they would change. Listen more than you talk and avoid turning questions into criticism.
- Record observations against each theme: Note what you saw and heard under safety, quality, standard work, 5S and flow while it is fresh. Capture facts and specific examples rather than general impressions, because a vague note is impossible to act on or follow up later.
- Agree actions and follow up: Turn observations into a short list of follow-up actions, each with an owner and a due date, and confirm them with the team before you leave. Review progress on those actions at the next walk so the loop from seeing to fixing is closed.

Gemba walks work best as a routine rather than a one-off. Many operations run a short daily or weekly walk of a priority area by frontline leaders, with a broader walk by senior leaders monthly. Consistency matters more than length, so a focused fifteen minute walk done every week beats a long walk done once a quarter.
Rotate the area and theme so the whole operation is covered over time and no part of the floor is walked so often that it becomes a performance for the leader. Always close out the previous walk's actions before or during the next one, because walks that never lead to change quickly lose the trust of the people on the floor.
