What is a confined space management plan template?

A confined space management plan is the strategic site-wide policy and procedure document that governs every confined space on a property. It sits above any individual entry permit or per-space risk assessment and defines how the duty holder identifies registers controls and manages confined spaces across the entire site, who holds each role, what competency is required, how rescue is arranged and how the program is audited. The plan is the upstream governance document an Australian PCBU must hold under WHS Regulations 2011 Chapter 4 Part 4.3 (confined spaces), AS 2865-2009 (Confined spaces - clauses 2.1 and 2.2 management responsibilities), Safe Work Australia Code of Practice on Confined Spaces, OSHA 29 CFR 1910.146 (Permit-required confined spaces - paragraph (c) employer duties) and UK Confined Spaces Regulations 1997 with HSE L101 Approved Code of Practice.\n\nThe management plan is distinct from a per-space risk assessment (which scores hazards for one space) and from a confined space entry permit (which authorises one specific entry). How often should you complete this plan?

The confined space management plan is a controlled WHS management system document with a fixed review cycle plus event-driven triggers. AS 2865-2009 clause 2.2 and the Safe Work Australia Code of Practice for Confined Spaces expect the plan to be reviewed at least annually. OSHA 1910.146(c)(7) requires the written program to be reviewed within one year of a permit being issued and after any unauthorised entry detected change in operations or near-miss.
