What is a handover document template?

A handover document template is a single signed form that records the moment responsibility for an item passes from one party to another. It captures what is being handed over and its identifiers, the direction of the transfer, the condition at the point of exchange, the accessories, keys and documents that go with it, any outstanding defects, and the names and signatures of the person releasing it and the person accepting it. How to use this handover document template

  1. Prepare the record before you meet: Fill in the item details, asset ID and the reason for the handover in advance. Pull the previous handover record and the service history so both parties can see the condition the item was in the last time it changed hands.
  2. Walk the item together: Inspect the item side by side rather than separately. Agreement is far easier to reach when both parties are looking at the same panel, and a fault raised at the point of exchange costs a conversation rather than a claim.
  3. Record condition and take photographs: Note the meter reading, fuel or charge level and every mark, dent or fault you can see. Photograph each item of damage from a distance and close up, and reference the photo numbers against the lines in the report.
  4. List accessories, keys and documents: Tick off attachments, keys, chargers, manuals, certificates and service records one at a time against the form. Missing items are cheap to resolve now and expensive to chase once the item has left the yard.
  5. Agree outstanding actions and sign: Write down any defect that has not been rectified, who is responsible for it and by when. Both parties then sign and date the form, and each keeps a copy so neither side is relying on the other for the record.
  6. File it against the asset: Store the signed handover with the asset record rather than in a job folder. Over time those dated condition entries build a custody history that shows exactly which period of use each fault appeared in.

How often should you complete this handover?

Complete a handover document every time responsibility for an item moves, including internal transfers between sites, crews or shifts. The transfers people skip are usually the internal ones, and those are exactly the moves where damage stops being traceable to anybody.

For hire and loan items, complete one at issue and a matching one at return so the two records can be compared line by line. For long term allocations, repeat the exercise whenever the holder changes or at a set review point, so the condition record never gets more than a few months out of date.

ISO 55001 expects controlled, traceable records covering the custody and condition of assets, and ISO 9001 has a specific requirement around property belonging to customers or external providers: it must be identified, verified, protected and any loss or damage reported. A signed handover document does both jobs at once. It shows what was handed over, in what condition, to whom, and on what date, which is the evidence an auditor asks for when they want to know how you control assets that leave your direct supervision.

A delivery docket confirms that goods arrived at a place, and its focus is quantity and timing. A handover document confirms that responsibility for a specific item passed to a named person, and its focus is condition and custody. A docket answers what turned up. A handover answers who now owns the risk and what state the item was in when they took it on. Where a transfer involves an asset with condition that matters, use the handover form and attach the docket to it.

Yes. A record signed by only one side is a statement rather than an agreement, and it carries very little weight when the condition is later disputed. Both the person releasing the item and the person accepting it should sign and date the form in each other's presence, and each should keep a copy. If the accepting party disagrees with a condition note, record the disagreement on the form and have both parties sign that instead of leaving it unresolved.

Photograph all four sides of the item plus the meter reading, then take a close up of every mark or fault you have noted. Numbering the photographs and referencing those numbers on the form is what makes them useful later, because an unlabelled folder of images does not tell anyone which handover it belongs to. If a fault is functional rather than visible, a short video is usually more convincing than a still.

At the point of exchange, with both parties present, and never afterwards from memory. A handover completed later is a reconstruction, and the whole value of the document is that it captures agreement at the moment the item changed hands. If circumstances mean the accepting party cannot be there, record that on the form, note who inspected it on their behalf, and get the record confirmed in writing as soon as they take possession.
