I declare that I have completed this wellhead inspection in accordance with API 6A requirements and the site integrity management plan. I have recorded all findings accurately and reported any defects or anomalies to the well operations supervisor and the integrity management team. Any critical defects affecting pressure containment or well control barriers have been escalated immediately. I understand that failure to complete inspections or report defects may breach regulatory obligations and the facility safety case.
