Declaration
I declare that I have carried out an inspection of the above work equipment in accordance with PUWER 1998, Regulation 6. I have recorded all defects observed. Where any defect presents an immediate danger, the machine has been taken out of service and the responsible person notified. This record shall be kept until the next inspection under Regulation 6 is recorded.
