Declaration
I declare that I have carried out a thorough examination of the above lifting accessory in accordance with LOLER 1998, Regulation 8. I have recorded all defects observed and, where a defect presents an existing or imminent danger, I have notified the duty holder immediately and will report the matter to the relevant enforcing authority as required by Regulation 10. Any accessory marked for destruction has been rendered permanently unserviceable.
