Declaration

I declare that I have completed this loading dock inspection before dock operations commenced for this shift. I have recorded all findings accurately and reported any defects or non-compliant items to my supervisor. Dock bays with critical defects (non-functional levellers, restraints or lighting) have been locked out until repaired. I understand that failure to complete an inspection or report defects may breach company policy and WHS obligations.
