Isolation

6Circuit breaker/isolator switched to OFF position
7Personal padlock applied to isolating device
8Danger tag attached with name, date, and reason
9Additional isolation points secured (all sources)
Proving Dead
10Voltage tester proved on known live source before testing
11Tested phase-to-phase, phase-to-neutral, phase-to-earth
12Voltage tester proved on known live source after testing
13Circuit confirmed dead at point of work
14Earthing applied where required (HV or long cable runs)
Work Completion
15All work completed and connections secure
16Tools and test equipment removed from work area
17Covers and enclosures refitted
18Insulation resistance tested (where applicable)
Re-energisation
19All personnel clear of equipment and warned
20Temporary earths removed
21Danger tag and personal lock removed by owner only
22Circuit re-energised and operation verified
23Affected personnel advised circuit is live

Declaration

I confirm that safe isolation has been achieved following the prove-test-prove methodology. The circuit was confirmed dead before any electrical work commenced. Only a licensed electrician shall perform or supervise electrical lockout/tagout.
