Critical Lift - Enhanced Planning Required

This lift meets one or more critical lift criteria under AS 2550 Section 8. Engineering calculations, dual approvals and a specific risk assessment are mandatory before this lift may proceed. No element of the lift may commence until all sign-offs on page 3 have been obtained.

Critical Lift Classification

Tick all criteria that apply to this lift. At least one must be ticked for this form to be used.

Tick Criterion
Load exceeds 80% of crane rated capacity at the planned radius
Tandem lift (two or more cranes)
Load travels over occupied buildings or public areas
Lift within minimum approach distance of live overhead power lines
Load has no certified lifting points (engineered or non-standard item)
Personnel riding on the load or hook block
Other (specify below)

Site Hazards and Controls

Tick Y (yes), N (no) or N/A for each item. All items marked N must be resolved before the lift proceeds.

# Item Y N N/A Control / Notes
1Ground bearing confirmed by engineer for crane and outriggers
2Outrigger pads / mats sized per engineering calculation
3Overhead power lines: safe approach distances confirmed
4Underground services located and marked
5Adjacent structures clear of boom swing and load path
6Exclusion zone barricaded, signed and marshalled
7Occupied areas below load path evacuated or shielded
8Wind speed confirmed within plan limit
9All rigging inspected and within WLL and inspection dates
10Communication tested between all key personnel

Abort Criteria

The lift must be stopped immediately if any of the following occur: wind speed exceeds ______ km/h; load spins or swings beyond ______ degrees; ground settlement or outrigger sinking observed; crane alarm activates; communication lost between operator, dogger or lift supervisor; any person enters the exclusion zone; or the lift supervisor calls a halt for any reason. If aborted, the load must be lowered to the ground or secured before personnel approach.

Declaration

I declare that this critical lift plan has been prepared in accordance with AS 2550 Section 8 (Special lifting applications) and BS 7121 (Code of practice for safe use of cranes). The load weight has been verified from a reliable source. Engineering calculations for crane stability, ground bearing and rigging loads have been completed and reviewed by a qualified engineer. All rigging components are within their rated working load limits and current inspection dates. Site hazards have been assessed, exclusion zones established, and abort criteria defined. All personnel have been briefed on the full lift sequence, hold points, communication protocols, abort criteria and emergency procedures. This plan must be revised, re-calculated and re-approved if any conditions change.
