Declaration

I have recorded all findings accurately and reported any defects or non-compliant items to the toolpusher / OIM. The BOP will not be returned to service until any faults affecting well control capability have been rectified and re-tested. I understand that failure to complete an inspection or report defects may breach company well control policy and regulatory obligations.
